Personal Information Collection Statement
This statement provides you, as the data subject to whom the data user, Happy State Counselling and Development Centre (Happy State), provide services, with notice as to why your personal data is collected, how it is intended to be used, to whom the Data may be provided to, and how to access, review and correct the Data, in accordance with the requirements of the Hong Kong Personal Data (Privacy) Ordinance (“PDPO”).
The Personal Information Collection Statement (PICS) is a statement given by Happy State to be presented to the individual, the data subject, on or before collecting the personal data from that data subject, whether known or unbeknown (e.g. through the use of cookies).
Purpose of Collection
The information collected from you will be used by Happy State for all purposes related to your use of Happy State's services.
Voluntary to Supply Personal Data
Provision of personal data by you is voluntary. When you provide personal data to us, please make sure that the data is accurate and complete. In the absence of your accurate and complete personal data, we may not be able to provide you with the services you require or it may also result in being deferred or rejected.
Disclosure of Personal Data
The data collected will be kept in confidence. We will not provide your personal data to third parties for any purposes without your consent.
We may disclose data when required by law or other government and regulatory authorities; or if explicit consent to such disclosure is given by the data subject.
Retention of Personal Data
The Data will be retained for as long as necessary to fulfill the purpose for which the Data was collected. The Data which is no longer required will be destroyed. You have the right to access and request for permanent deletion of any personal data collected and stored by Happy State.
Rights of Access and Correction of Personal Data
You have the right to request access to and correction of information held by us about you. If you wish to access or correct your personal data, the request for accessing personal data must be made in writing.
Changes to this Personal Information Collection Statement
Happy State reserves the right to update this Personal Information Collection Statement at any time and will make available an updated Personal Information Collection Statement on our website or in writing so that you are always aware of what information we collect, how we use the information and under what circumstances the information is disclosed. Any such change, update or modification will be effective immediately upon posting.
If there is any inconsistency or conflict between English and Chinese version of this statement, the Chinese version shall prevail.
